Pull out the IAC block off the air bypass holes and if it still idles you definetly have a vacuum leak. A blown or stuck open PCV can cause a fast idle too. If not obvious I use an IAC driver tool to check it's operation then from there the connects to PCM but rarely find wiring the problem.
Sounds like a bad O2 heater. Resistance on the heater side of O2 should be 4-5 ohms...and with it disconnected key on Battery voltage to the heater circuit on cold engine. The heater warms up O2 so that they work faster on cold engines to control mixture.
Usually when the switch is bad the speeds don't work. Sounds like the blower motor resistor block, which is in the HVAC box by the blower motor for cooling, under the hood. This is what changes the motor speed by changing the voltage to the blower motor. You can test the switch changing speeds...
